​1) Sign up as a team by clicking on the link below!

2) Download and complete the registration form and indemnity form below (for participants below the age of 21 as of 1st January 2013) latest by the registration period on the event day itself. Failure to submit the indemnity form would render the player to not be allowed on court.

3) Submit a hardcopy of the above forms and registration fees to the person-in-charge at McDonald's, Raffles City on any of the following dates between 12pm-6pm

For Early Bird Package

1 June 2013 (Saturday)
2 June​ 2013  (Sunday)

​ 

For Normal Package​

22 June 2013 (Saturday)
23 June 2013 (Sunday)

 



4) All entries and payment must be completed by 23rd June 2013 or before the specified period as stated below to be entitled for the early bird discount.

5) No E-invoices will be issued, but receipts with the NUSSU letterhead will be given after your payment.

​

6) *NEW* Walk-up Registrations are now allowed @Raffles City!

All you have to do is to come down on the collection days (1 & 2 June and 22 &23 June) with enough cash and sign up straight away with us!  

(Kindly note that parents of participants below 21 are still required to fill in the indemnity forms.

This could be submitted either on the collection day or during registration on the event day itself.)
